What’s Included
Second Visit
Meet one-on-one with your physician to go through the results of your extensive lab work, review your personalized weight loss plan (Phase 2), and discuss how to best treat any hormonal or metabolic imbalances. Typically, patients move to Phase 2 on their second or third appointment.

This appointment is approx. 20-30 minutes. During this time, you will review the following:

Your Biochemical Composition
• Review the results of your extensive lab work, focusing in on your specific biochemical make-up and biomarkers that are preventing you from losing weight or keeping it off.
Your Personalized Plan (Phase 2):
• Review your new, personalized weight loss plan. Your Phase 2 plan is tailored to you based on the results of labs and comprehensive health bio. through food chemistry and addressing your specific biomarkers.
• Based on your biochemical composition, medical history, and food preferences (intolerances, allergies or sensitivities), your physician works to reset imbalances through food chemistry and supplements if needed.
• In order to ensure maximum weight loss and optimal health, plans are tweaked during follow-up visits as needed.
Third Visit (Follow-up Appointment)
Meet one-on-one with your physician to weigh-in and track your success on program. In each virtual follow-up appointment, you will meet with your doctor one-on-one to review your progress and make adjustments to your program to maximize weight loss, if needed.

• Outcome-focused: Depending on patient need, follow-up appointment can be as short as 10 minutes or as long as 20 minutes. We are here to support you get you to goal weight by empowering you with the tools, resources and knowledge for success all while being as convenient as possible.

• Convenient: Losing weight does not require you to give up your Sunday morning to go sit in a dusty, old church basement and listen to people talk about problems that do not relevant to you. We (virtually) come to you and work with you to get the outcomes you desire. And no, we are not offended that you’d rather spend time chatting with friends that chatting with us. We are laser-focused on our mission to get you to goal weight, and to optimize your health.
Post Visit Curriculum
After your second and third visit, you will receive curriculum and chef-designed recipes as appropriate and as desired.

• Empowerment through education: We strongly believe in empowering you with the knowledge, skills and tools to get to your dream weight, and stay there for good. In order to achieve this goal, you need to understand the science and knowledge behind weight loss.

• When you understand how the food you put on your plate and into your body sets you up for success (or not), you're free to make the best choices for your body.

• No need to obsess over every meal or count calories. (We don’t do that here). Through our curriculum, you will learn a structure that works best for your body. With this knowledge, you can make decisions to ensure you feel full, elongate your blood sugar curve, and have the nutrients you need to feel great throughout the day.
Terms of Service
• Fee includes your second and third visit with physician as well as educational material. If needed, supplements, medication and food not included.

• All packages are non-refundable. Should you decide to discontinue with the program for any reason, packages cannot be refunded for appointments or curriculum not used.

• Both appointments must be scheduled within 5 weeks of purchase date.

• All packages are non-refundable. Should you decide to discontinue with the program for any reason, outstanding appointments or curriculum cannot be refunded as packages are final sale. Changes and cancellations must be done 48h prior to appointment. Changes made within 48h will be charged a $30 fee. Changes made within 10h will be charged 50% of appointment cost.
