We are excited to work together. Let's jump into our program policies.

Your monthly membership includes visits with your medical team. Based on your membership tier, each month, you have a set number of visits with your MD/RD team.
All appointments must be completed within the current month. Appointments cannot be 'rolled over' to future months.
Membership plans auto-renew each month until cancelled. We require 5 business days notice for all membership cancellations.
To cancel, simply send us an email or login to your portal and cancel your membership. Letters and phone calls are not permissible.
Additional appointments are available at discounted rates as long as your membership is active.
If you need to restart after becoming inactive, you will be billed at the initial consultation rate.
We do not accept insurance, but out-of-network reimbursement assistance is available. To learn more, please inquire about our partnership with our medical billers to create Superbill forms.

We require 48 business hours notice for all appointment changes.
A $35 fee applies for cancellations made between 24-48 hours before your appointment.
You will be charged the full appointment cost for cancellations with less than 24 hours notice.
You may cancel your appointment through email, our online portal, or by speaking with a receptionist. We cannot accept cancellations through voicemail.
Rescheduling within the cancellation window will still incur applicable fees.

Your care and comfort are our highest priority. We record sessions to provide you with the best possible medical care while ensuring your provider can focus fully on you during visits.
Sessions and phone calls are recorded to enhance patient care and note-taking accuracy.
Participation in telehealth visits implies consent to recording unless previously opted out.
Recordings are stored locally with no external party access.
All processing occurs locally (Compression, ASR, OCR).
Recordings may be used for: Clinical and medical note-taking, Service improvements, Product development, Training purposes and Data assessment and analytics.
Patients must notify Diata Health if they wish to opt out of recording.
We maintain strict security measures to protect recordings.
Your privacy is paramount - we handle all recordings with the utmost confidentiality and care.

We safeguard all protected health information according to federal law.
We only release information through channels you have authorized.
You may choose to receive communications through home phone, work phone, cell phone, text messaging, or email.
You may authorize third parties to receive your information.
We only leave messages with verified recipients.

We require a credit card to be kept on file for all active members.
All payments are due at the time of service.
Your card may be charged for services, late cancellation fees, and additional appointments.
You must be 18 years or older to make purchases.
You may update your payment method at any time by email or phone.
Unpaid Balances:
We will notify you of any outstanding balances via email. Any unpaid balance after 15 days will incur interest at a rate of 1.5% per month (18% APR).
You are responsible for all costs associated with collecting overdue payments, including legal fees.
Accounts with unpaid balances may be suspended until payment is received.
Multiple late payments may result in a requirement for prepayment of services.

Promotional offers are subject to available inventory.
You cannot combine promotions with other offers unless specifically stated.
Promotions do not apply to future or previous purchases.

Promotions cannot be redeemed for cash or credit.
Additional fees may apply for tax, duties, VAT, government fees, and shipping.
